These are the requirements:
1. 4 photocopies of the Child's Birth Certificate- in English and should be typewritten/computerized(issued from the maternity hospital) translated in English
(The address should be simplified for example: I-Sarang OB GYNE Clinic, Busan, Korea --
2. 4 photocopies of parents passport(1st page and last)
3. Marriage contract (4 photocopies)
4. 3 passport size Pictures (royal blue background- no hats, wearing shirts with collar and should comply with the standards of applying the regular passport- pls. refer to the DFA homepage for more info about the picture)
We had a hard time doing this because we took Chloe's passport pictures in a photo studio in our neighborhood and we had it in white background. The new ID pict was so new last year August, 2008 so we never thought of the changes by then so we resorted to take Chloe's picture in the nearby Philippine store.I advised you to do it somewhere not so public so the child won't be so distracted to take picture unlike Chloe who was so sleepy at that time after a day spent waiting in the Embassy!
5. 4 Filled up forms of Report of birth [FA form no. 40]
all forms should be original and no, as in NO ERASURES)- you can download the form at the PH website, and type the info and print it... be sure it's the 0 legal size
This was the hardest part, I had filled all forms legibly (because there was no downloadable forms that time)with the best of what I could with all those 4 pages and they were rejected by the officer in the counter because of the address! Make it short and simple: Busan if it is Busan.. and then I've had too many mistakes because we were on the rush because of the lunch time nad the closing time of the embassy. It was summer then that's why the place was so crowded with people from far places and the officers were too busy to entertain too many questions at that time! Foot prints are needed at the back of those 4 forms and you need to find 2 filipino witnesses. It would be nice if you're gonna ask for the form beforehand and ask your chosen god mother/father to do the signing- in Chloe's case I asked two filipino in the PH embassy- if a parent would not be present at the time of application, he/she should sign the forms beforehand.
You should take caution of the following entries:
Place of birth and date of Report (located at the upper right corner of the form)- NEVER FILL IT UP it will be stamped by the PH Embassy
Place of birth(in full) ... (make it simple... e,g, I-Sarang OBSTETRICS & GYNECOLOGY, Busan, Korea)
Name and address of physician/nurse (make it simple too e.g Dr. Jeong Min Kim/ I-Sarang OB GYNE, Busan, Korea)
I would advice anyone to visit the embassy before filing, ask for the forms (take some extras in case you'll have mistakes) and ask for the detailed info. I like the man who is in his mid-40s, a little short, chubby and I think, who has this mustache. He tells patiently in details on how to fill up the forms.
After filing the forms (including the passport application form) you'll get your personal copy of the Child's report of birth right away (one of those original copies which you have filled up) but it will take time for the NSO to register the report of birth. Last year, the officer said that the NSO has a Backlog and there were cases that they have never registered it at all so it would be advisable to request for an autheticated NSO copy after 6 months of application to make sure they received the application. The passport will be released after a month though.
Fees:
26,325 won for the Report of Birth
56.650 won for the New Passport
(always prepare the exact amount so you don't need to wait)
